Case Study: Covenant Occupational Care

On 10/24/16 I met Mr. Russell at the Covenant Occupational medicine clinic. Mr. Russell reports his pain remains a 1-2. He said when he bends or twists his pain will increase. As we waited to be seen Mr. Russell changed positions frequently. He has completed the Functional Capacity Evaluation. Recommendations are for him to attend 6 2 hours sessions over 2 weeks of a work hardening program. This service is pending approval by the adjuster. Mr. Russell is only taking Motrin for pain at this point.

On 11/3/16 I met Mr. Russell at the Covenant Occupational medicine. Mr. Russell has been in the work hardening program for 3 sessions at this point. I waited over an hour when due to another appointment commitment I had to leave. I did request
