Back To Work Case Study

This counselor met with Steven in the office today (12/13/17) to get an updated paystub showing he is back to work, a clearance from his doctor, and to review his budget check-list. A copy of his paystub as well as his social security card are in his case file.

The doctor's note states he is able to return to work full time with light duties. The paperwork states that Steven has lifting restrictions. He is unable to lift anymore then 2.5lbs with his right hand with his elbow at his side. This is not an issue for Steven as most of his work is at a desk and doesn't require lifting. Steven was released to work on 11/20/17.

After reviewing all the paperwork, this counselor explained the new AudioNet process to Steven. His hearing test and clearance
