Mrs. S was in need of extensive rehabilitation in order to return to her baseline functioning and return to work. Intervention: The case manager worked closely with her healthcare team and arranged for timely transition from acute care to their network participating rehabilitation unit. When able, she was transitioned to a three times weekly outpatient program. Prior to returning to work, the case manager arranged for the occupational therapist to visit the patient's employer and evaluate her workstation and duties for safety in light of her minor residual limitations.
