SUMMARY
Mr. Jetke reported that he has been working his regular job delivering fire extinguishers. It was a windy day and the rear door hit him on his right side. Mr. Jetke had achy pain to his right side. Jetke was evaluated by Dr. Scott and completed seven sessions of physical therapy. Mr. Jetke felt that his mobility has improved compared to prior weeks and rated his shoulder pain at a “6” when not in use, but when he uses his right arm, he rates his discomfort a “10” on the 1-10 pain scale. Mr. Jetke was unable to sleep on his right side so he sleeps intervals and propped up. Mr. Jetke had an increase in his numbness and tingling sensation in his right arm, which radiates into his fingers especially the index finger. Dr. Scott referred Mr. Jetke to a Neurologist for an EMG. Dr. Scott thought the pain was from the C5. The physician felt that the multiple findings would relate more to degeneration of the soft tissue of the left shoulder as well as osteoarthritis being related to degenerative changes of his shoulder related to aging process. The physician didn’t feel that the incident that Mr. Jetke described would cause these multiple changes of the right shoulder. The physician recommended that Mr. Jetke continue under the care of Dr. Scott and that he didn’t feel that the surgery recommended would be related to the work
