Based on the doctor’s first report of occupational injury dated 10/31/16 by Dr. Feldsher, the patient complains of tingling in both hands, right greater than the left side.
On examination of the bilateral wrists, range of motion shows flexion is 30 degrees and extension is 45 degrees. Tinel’s and Phalen’s tests are positive, right greater than the left side.
Patient was diagnosed with bilateral carpal tunnel syndrome, myospasms and bilateral trapezius strain.
She was dispensed with Medrol Dose Pak, Norflex and Biofreeze.
