Dr. Black Observation

On 3/26/18 I met Ms. Black at the office of Dr. Sardelli. She reports that her shoulder is still painful; she cannot fully extend her elbow or rotate from side to side. She reports pain to the inner aspect of the elbow. Her 4th and 5th fingers cannot open fully or close fully. There continues to be a pain in the inner elbow. She is still working with the hand therapist. Dr. Sardelli said that while she was in the hospital he did do a manipulation under sedation of the right shoulder. Today he has given her a cortisone injection. He said he doesn’t do cubital tunnel releases which is what she needs of the elbow. He recommended Dr. Ohm whom Ms. Black has worked with in the past. I have called Dr. Ohm’s office and they will call me back to set up the appointment. …show more content…
Ohms, Dr. Quinn then Dr. Fallahi. There either were no appointments until June or the provider recommended by Dr. Sardelli would not accept the insurance. I have provided Dr. Fallahi with prior medical records.

On 4/9/18 while I was en route to meet Ms. Black at the office of Dr. Fallahi she called. Ms. Black reports that she was not picked up by Swartz to be taken to her appointment. I made a call to Swartz and was told they do not transport out of Genesee County. I had made arrangements for transportation on 3/28/18 and was not told that. I made contact with Dr. Fallahi’s office and rescheduled her appointment. I have the adjuster approval made arrangements for Alomar transportation.
