The claimant’s two Supervisors, Mr. Chea Choe and Ms. Maria Ramirez found the claimant to be one of their “best Food Packers in the Food and Pantry Department,” where the claimant has been assigned to since her date of hire on 11-18-2011. They claim the claimant has not had any medical issues or concerns throughout her employment as they have not received any medical work status reports or doctor slips involving any non-occupational or occupational related injuries or illnesses, until 12-17-15.

Choes incident report and the claimant’s allegation of her claim, Mr. Chung processed the claimant’s claim and offered the claimant medical treatment at the Insureds pre-designated medical provider at Express Medical Clinic on that same day.
Mr. Chung alleged the claimant did not have any loss time off from work after the claimant returned to work that same day on 12-17-15 with a Doctor’s First Report of Injury or Illness, that specified that the claimant was now alleging pain to her right elbow that radiated to her fingers with numbness and tingling. Mr. Chung claimed he accepted the new changes of the diagnosis and hence accommodated the claimants modified work restrictions imposed of limited lifting to 15 pounds and no overhead reaching above shoulder level.
Before interviewing the claimant, we discovered a discrepancy between the claimant 's initial version of the injuries she reported to her supervisors and the Insured. We found that the claimant never mentioned or complained about having any pain or discomfort which supposedly started in the claimant’s right shoulder as the medical report specified. Furthermore, there was no mention or complaint about any pain or discomfort to either of her wrists to the treating physician, compared to what the claimant said to Supervisors Ramirez and
