Basic Information

Mr. John Smith is a 73 year old white male who lived in Hixson, TN for 47 years. Mr. Smith was admitted to our facility from Memorial Hospital for skilled care services. In March, he experienced a decline in his condition, due to several falls. Mr. Smith appeared to look confused, weak and upkeep upon his arrival. During the assessment interview, several questions had to be repeated and time had to be given to allow Mr. Smith to reply. He stated, “My wife and children handle all of the finances.” Mr. Smith states also, “I want to return home after, I complete rehab.”

Mr. John Smith is currently married with two children. Mr. Smith reports that he grew up in a household with his mother, father and his siblings. Him being the oldest. Mr. Smith reports that he and his spouse have been married for over

If not eligible, BWS will arrange for Mr. Smith to receive Home Health Service which will include physical, occupational and speech therapy, along with a certified nursing assistant.

Intervention 3 ~ BWS will arrange for Mr. Smith to have a hospital bed (if needed) and wheelchair.

Smith’s assessment. First, Mr. Smith meets the requirement for Role Theory due to the fact that he would like to return home, so that he can take back on the role of being a husband to his spouse.
