The healthcare industry is an imperative one as it deals with lives. It is a vast industry, according to Morgon (2014) that includes the integration of goods and services with the aim of treating patients, preventing diseases, rehabilitation as well as the provision of palliative care. On the other hand, according to Smith (2015), patient-centered medical home is a place and a model that shows a way of organizing basic care communication and coordination of primary care to the preference of the patients. It comprises of five functions that include: quality and safety, coordinated care, accessible services, comprehensive care and patient care (Smith 2015). Each of the duties has in such a way that it works in conjunction with others.
