Focus: MS. Smalls (MHP), Mr. & Mrs. Bryant (MHS) and Val review Val’s Individual Plan of Care (IPOC).
Intervention: MHP, Val, and MHS discuss transferring to a new school after being enroll for only a month. MHP, Val and MHS review IPOC goals and interventions. MHP answered questions and provide examples of possible positive interventions. MHP, Val and MHS sign the IPOC and MHP provide a copy to Val and MHS.
Response: Val smile as he enters the office. MHS explained Val will attend the school in the community and request documentation from the previous school.
