Focus: MS. Smalls (MHP), Mr. & Mrs. Bryant (MHS) and Val review Val’s Individual Plan of Care (IPOC).
Intervention: MHP, Val, and MHS discuss transferring to a new school after being enroll for only a month. MHP, Val and MHS review IPOC goals and interventions. MHP answered questions and provide examples of possible positive interventions. MHP, Val and MHS sign the IPOC and MHP provide a copy to Val and MHS.
Response: Val smile as he enters the office. MHS explained Val will attend the school in the community and request documentation from the previous school. Val asks questions about the goals and interventions. Val and MHS sign and receive a copy of the IPOC. MHS report Val is adjusting to the home. There was one incident where Val stayed
