On 7/5/2016, CM met with the client to complete Bi-Weekly ILP Review. In the meeting client appears to be upset and distraught. Client mood was un-balanced and her affect was in- appropriate. Client denied suicidal or homicidal ideation. Client was dressed with proper attire for the weather.

SOCIAL SUPPORT UPDATE: During the meeting the client was complaining about Shift Supervisor Shawn removing her personal belongings from her bed. CM along with the client met with shift Supervisor Shawn/ADL’s who stated the client was giving numerous notes stating that she has excessed belongings under her bed and she needed to either put it in Storage or leave it with family members. Shift Supervisor to report client. Client failed to comply and her belonging is temporarily was removed

CM reiterates the Shelter Rule and Regulation and the Code of Conduct and for the client to adhere to the shelter 10pm curfew.

WORKER ASSESSMENT: Client personality appears to be aggressive, demeanor and hostile. During the meeting session with CM client was clam and cooperative. CM only observed sign of hostility when client was speaking with the Shift Supervisor. Client also reported she is ready to be housed and she will like to be transferred to onsite HS.

Plan of Action: Client must participate in all scheduled meetings, client must meet with CM every Thursday, client must participate in all off-site and on-site medical appointments, Client must continue to attend outside mental health counseling and submit monthly proof of attendance, client must accept first suitable housing offers, client must adhere to the shelter rule and regulation including 10pm curfew and client must submit a copy of her birth certificate. CM reviewed the Initial ILP, client agreed and signed. Next ILP Review is scheduled for
