This is Cn first scheduled meeting for IC. Th met with Cn in his home. Th reviewed with Cn and Cn caregiver, therapist scope of practice and informed the client and caregiver that Th is pursuing licensure and receiving supervision from a fully licensed clinician. Th also reviewed confidentiality, and its limitations, mandated reporting and requirements. Th discussed with Cn, cn strengths and desires for treatment, reviewed IRP, and collaborative documentation. Th completed and reviewed with Cn a safety plan due to Cn having history of self-harm thoughts.
