The client has met with counselor to completed his discharge planning paper work. The client had met all the requirments of his treatment plan and the program. The client while being a part of the program appeared to actively working on his family issues by attending the family group sessions here at the Quest House. The client also appeared to learning new tools relapse prevention tools. At this the client will not be going to sober living his will be living with his grandmother. The client reported that he will be attending AA meeting with a family member for moral support. At the end of his program the client appeared to working on gaining self awareness.
