Communication is important in every clinical setting. Effective communication plays the vital role in the initial health screening as it eliminates possible misdiagnosis and inappropriate follow up (Sonethavilay et al. 2011). I helped the collaborative team in appointment setting, writing clinical notes; writing the appointment letter and calling and referring to different services. I have communicated effectively with the GP clinics, Girl and Women health series, Clients. For the non speaking client, I have used the Telephone interpreter service and interpret impersonally to deal with a client. This practice helped to be more confident in the both oral and written communication skills which I will apply in other setting in …show more content…
Contradictally the care provision is done in the clinic where the client and family member have appointment. I have understood that the immunization, vision/hearing, dental ,developmental signs are initial health assessment for child while health including TB ,mental health, immunization ,vision /hearing and deaf are the initial health assessment for the adult. Then the client care of provision on maternity, sexual and reproductive, diet, vitamin smoking and immunization and dental is carried in the following stage. I have developed comprehensive and systematic nursing assessment in the initial screening of the clients. This assessment consists of taking the medical history, physical examination, psychiatric history, psychosocial assessment techniques to collect data. Mental health assessment of the client is done by assessing the client’s behavior, mood, appearance affect and speech. We send the clients to different referrals if necessary with appointments. The appointment remainder in their client‘s language is translated if the client is unable to read English. We follow up the patient where they have attended the referrals and enquire about their health status as well. We also referred to the STARRT services to those who need it. I learnt to be culturally sensible and showing empathy to the client who have the effects of trauma and torture, injuries and
