Eastgate Springs Nursing Facility: Case Study

SSA participated in Care Conference Meeting at Eastgate Springs nursing facility. Also present was James, his wife Elizabeth, and Chris with Social Services, Brittany with Nursing and Susan with Molina and Mark with Occupational Therapy. Mr. Castle’s son and Power of Attorney, Jameson, was not able to attend the meeting. The purpose of the meeting is to discuss discharge from the nursing facility; Mr. Castle has been expressing a desire to return home with his wife. Brittany with Nursing shared with the team that Mr. Castle is currently a two person transfer, when he is willing to transfer, and is a difficult transfer for two individuals. She shared that there have been no concerns with him taking his medications. She shared that he is unable …show more content…
The team discussed and Mr. and Mrs. Castle agreed that he would continue to participate in OT services at which time he is ready to discharge from services or plateaus in progress the team will come together to review if Mr. Castle is ready to return home. Mark also reminded the team that the last time Mr. Castle was discharged from the nursing facility, which was an estimated two months prior, it was the recommendation of the nursing facility that he receive 24 hour assistance. Mr. and Mrs. Castle shared that when he was home during those two months they felt he was doing well with supports from PASSPORT, however he became ill with pneumonia which placed him back in the hospital and then returned to the nursing facility. Susan with Molina shared that she cannot speak to assistance that may be available in the home until a discharge date is set. SSA spoke to services Mr. Castle currently receives through Clermont County DD. Chris spoke to the recommendation that long term place is recommended, however if Mr. Castle choose to discharge the nursing facility would contact Adult Protective Services and shared that they do not recommend his return to the community at this
