On this date APS supervisor, Matthew Mitchell and Jamie Gaines, APS social worker, visited Shadescrest NH, for the purpose of monitoring Ms. Salter's placement and attending a care plan meeting. At the meeting was Ms. Salter, Ms. Kathy Harville, Ms. Bethany Tidwell, NH DON, and Kim Kelly, NH SW. Ms. Kelly and Ms. Tidwell stated Ms. Salter's physical situation had improved. She is no longer in a Gerri chair and can wheel herself up and down the hallway in a wheelchair. She does require some assistance with ADLs and a Hoyer lift when transferred from the bed to the wheelchair. She no longer requires a catheter. .
Ms. Salter stated she wanted to go home and would like the department to set up another court date.

    For care staff, during the day on each shift I arrange an assistant manager and two care assistants. I instruct the assistant manager to make a report on each resident, the report is written down in the daily report book. When changing shifts the report book is discussed with the assistant manager who is coming on shift.
