Care Conference Case Study

Care conferences are held for every person receiving health care at home or in a care facility. They help the “care team” ~ everyone involved in the person’s care ~ share information and work together to meet the person’s needs (1). In this case study, I am the LPN who is responsible to organize a care conference for three of the residents in the residential home that I work with. To organize this care conference, it is imperative that I will fully know the goals for each of the resident. I have to prepare every necessary documents that are needed, facts about the resident like their record when the first move in to the facility, physical and medical history of each resident, and the recent report of the resident. As the nurse of the conference I have to be empathetic during the conversation of the conference. Having said that. I have to be clear and concise on my response to every member. An example of an active listener is when I can paraphrase what a member said if it is not clear to me by saying: As I have understood, what you said is, it is important to keep the resident in the exercise regime three times a week. Clarification is another way to show that I am actively listening during the conference. Like saying: You stated earlier, that Mr. Resident 2 needs an exercise regime. Can you tell me more about what kind of exercise he can do? Lastly, at the end of each care conference, as the nurse who is actively listening to what everyone have said, I will be able to summarize the entire content of the conference.
