Circumstance: Ms. Smalls (MHP) made face to face contact with Ms. Cordova (caregiver) and Richard at the home.
Action: MHP, Ms. Cordova and Richard review monthly paperwork. MHP complete quarterly training and request for Richard to explain his understanding of the training. MHP and Ms. Cordova discuss the delivery of Richard’s new box spring and mattress. MHP expresses the importance of keeping his living are clean to avoid the bed bug issues. MHP, Ms. Cordova and Richard discuss Richard completing the monthly paperwork timely with assistance, Ms. Cordova express her concerns of Richard not communicating in the home. Richard expresses not wanting to do his paperwork and ask why MHP or Ms. Cordova cannot complete the paperwork on his behalf.
