This writer addressed with the patient about TEAM's decision of not reinstating him for treatment due to his non-compliance of his multiple AWOLs and following through with Hartford Dispensary with regards to making his payment to the clinic. According to the patient, he did everything on his end with the Hartford Dispensary, but the clinic wants him to be taper off on his methadone. On Wednesday at 12:03 AM Resident Director Brandon Winslow received a phone call from Resident Assistant Christian San Pedro regarding a resident of concern. Resident Maryssa Hodder and Resident Marena Horan came to Resident Assistant Kayli Boyes with concerns on the whereabouts of Resident Falon Nonemacher who also resides in McHale 327. Resident Nonemacher was not answering phone calls from both Resident Hodder and Horan. RD Winslow was informed that Resident Nonemacher left to go home on Tuesday October 3rd around 1:00AM. During this investigation, the Alive and Weill Check with Ms. Emma Hernandez took place during an announced visit with her at the La Posada retirement community center at 501 S. La Posada Circle, Green Valley, AZ 85614. The retirement community center is a private well-guarded facility with roaming Security Guards throughout the center which prohibits photos to be taken within the facility, as all visits must be through contacting the front lobby office and checking in before any contact is to be made with any of the tenants. Ms. Hernandez’s meeting was held in her apartment # 123, which was found to be a one bedroom apartment which had a small front living room and a small kitchen with a refrigerator and stove. It all began when I arrived to the camping site called Fusina in Venice, Italy on June 22, 2016. I happened to experienced mosquitos bites and didn't think much of it do to the nature of it and because I would of never thought of it being malicious. Then day two (June 23, 2016) in Venice, Italy; I noticed my mosquito bites where very large and in areas like my ankles it was getting worst and I was starting to swell.
