MSW intern followed up with case manager, Yolanda Vargas regarding ct/family needs and linkage to community resources as expressed by mo during intake session. MSW intern provided feedback to Y. Vargas regarding barriers in obtaining community resources and mo’s inability to come in for session with ct due to change in work schedule. MSW intern provided Y. Vargas with mo’s contact information and explained mo has fluctuating work schedule. MSW intern reported that mo has identified need for assistance with basic support and reported linguistic barriers which impact family’s ability to support ct’s academic functioning. MSW intern provided additional information regarding need for linkage to clothing for ct’s school uniforms. MSW intern informed case manager Vargas that a needs assessment and TCM goal were established with the ct/family (Ct/family will identify and access at least 2 community resources (basic support, educational, IEP, mentor, etc.) to help improve clt's functioning within the next 6 months)).

Response: Ct/mother were not present for consultation. Y. Vargas agreed to follow up with ct/family via telephone to schedule meeting and discuss TCM needs. Y. Vargas inquired about family needs and composition. Y. Vargas asked about family’s income and if there is a need to assist with food resources. Y. Vargas agreed to discuss existing community resources to assist with ESL classes and school supplies/uniform for ct/family.

Plan: MSW intern will assist with scheduling referral for case management services as needed. MSW intern will monitor referral/linkage with mo/fa in order to assure that ct/family access community resource in order to prevent further deterioration on the quality of
