On 10/13/2015, CM met with the client for Bi-Weekly ILP Review.

Social Support Update: In the meeting client was alert, and satisfactorily groomed. She maintains eyes contact. She appeared in good health. She was in no acute distress. She was alert, satisfactorily groomed, and casually dressed. She was cooperative and appropriate in the meeting. Her mood was balanced and her affect was appropriate.

Client reported she does have family members in the USA but she doesn’t know the whereabouts of her daughter. CM inquires how the client doing since the last Bi-Weekly ILP Review. Client mentioned she wasn’t feeling well. She was having a lot of headache and she takes homemade remedy. A lot of mental, ginger and lemon tea.

CM along with the client called Ms. Brown/HRA CM tel# 929-221-3536 for update information concerning client's case. CM was unable to leave messages on the answering machine. The answering machine stated no messages it’s being accepted at this mail box at this time.
Client has an active PA case and Medicaid. Client can only work with limitation. She also reported she is not attending WECARE/FEDCAP she is waiting for letter.
Client reports she has a SSI court hearing scheduled for 10/16/2015 and Jose A. Diaz/Senior Paralegal at South Brooklyn Legal Services will be representing the client. Client reports Mr. Diaz want the judge to adjourn the day. Client report Mr. Diaz is requesting more day for client PCP to complete medical analysis.
Client continues to reports she suffers from, hypertension, Dyslipidemia, Psoriasis, H/O stroke, chest pain and this week she reports having headache and psoriasis. She takes the following medications: Lisinopril 5mg, and Hydrocortisone Cream 1%. Client reports she used to go to HCC "Health Care Choices" for all medical exams, but she has recently switched to AG "Ameri Health Group" located at 1349 Broadway, Brooklyn, NY 11221 718-975-2270 Dr. Dollis on/PCP and Dr. Vladititu/Cardiologist (client submitted copy of medical
