8/19/2015: The social worker was asked by the nurse to assist a patient, per Dr. R. Sundaram. The patient was requesting a letter to state his competence in supervision of his two children. The Social Worker is not qualified to address a letter such as this and consulted her supervisor about this matter. The Clinical Services Officer, Nursing Service Director, LPN – Team Lead, Dr. R. The reporting party (RP) stated approximately 3 weeks ago she visited with her relative, resident Harriet Kramer age 83 years. During the visit the resident was observed eating old, hard pizza. The RP described the resident as having dementia, osteoarthritis, and swelling of her arms. Conversely the resident was poorly groomed, had dirty hair, and smelled of dirty diapers. For care staff, during the day on each shift I arrange an assistant manager and two care assistants. I instruct the assistant manager to make a report on each resident, the report is written down in the daily report book. When changing shifts the report book is discussed with the assistant manager who is coming on shift. I give more responsibility to the senior staff, for instance, they give out the medication and are responsible for residents requiring a lot of care. It is a large organisation that provides help for medical purposes like broken bones, diabetes or even mental health and it is supported by the NHS. On 1/8/2016 SC met with Pa and PAS/son in Pa's home for home visit. Pa appeared properly groomed and dressed in street clothes. The home was unclean, and cluttered with urine odor nevertheless, all utilities appeared to all be in working order. Pa reported no, falls, hospitalization, change in health status, or medication. Volunteering at Little Light of Mine has continued to be a positive and rewarding experience. I continue to be impressed with the overview of the program and have learned a tremendous amount these past couple of weeks. The hands on experience I am having is truly remarkable.
