The CG reported that the Pa experience occasional memory lapse and difficulty with comprehension but is capable of making his own choice. The SC asked Gilda if she still the Pa’s primary informal CG and she stated “yes” and that her husband will help the Pa if she is unable to. So, the Pa’s informal remains intact and supportive. The SC reviewed transportation services with Pa and Gilda and they reported receiving CCT application; completed and mailed it back to CCT. A week later CCT mailed a letter to the Pa denying his application because he filled out the wrong application. SC will continue to provide assistance to the Pa to identify alternative community-based sources of transportation. The CG was told to follow the instructions on how to register with Logisticare to schedule to medical visits with the Pa. The using continues to use private transportation for now, but would like to use CCT. SC will contact CCT for the right application and have it send to the Pa. SC reviewed Pa's ISP. Pa confirmed that he continues to receive the service plan in the ISP in the following type, scope, amount, frequency and duration: Agency Model PAS 7 d x 3hrs daily from Sarah Care with niece, Giyani as PAS. Aide assists with bathing, dressing, grooming, transfers, all meal prep, light housekeeping, laundry and shopping. Pa also continues to receive ERS from Medscope, which he uses in the event of an emergency. SC will continue to see Pa based on the agreed upon contact plan of home visits every three months, calls every two months and annual reassessments. Self-directed service discussed, Pa declined as he is pleased with Agency Model PAS. Pa is receiving the amount of support necessary to ensure her health and safety needs are met and Pa remains satisfied
