Cronenwett et al. (2007) stated, evidence base practice is used to revise and modify policies to improve care. For instance, one of the meetings involved updating a current blood transfusion policy because it was noted verifying the patient with his wrist ID was not being performed, they were verifying the patient and blood with paper documents only. Also, nurses were not reporting a fever to the physician or to the blood bank. The new proposal was to notify physician and the blood bank of a fever or a reaction caused by the blood transfusion and to update how often vital signs are taken after the blood transfusion has been completed. Evidence demonstrates patients can still have a reaction up to a couple of hours after the completion of the transfusion. Updating the policy represents a system change because it will ensure more monitoring of the patient after he/she receives a blood transfusion. This concept applies to patient safety by ensuring the patient is being monitored in case he is showing signs of a reaction hours after the transfusion. This clarification prevented misunderstandings and prevent errors. Furthermore, my perception of being a manager is a difficult task. For instance, the role of a manager is difficult because they have to make appropriate decisions and oversee their employees in the specific unit. The manager I shadowed attended various meetings throughout the day, which she had to prepare before hand because each meeting was related to a different topic each time. She had to voice her opinions/ideas she had on each of the topics because she was in charge of incorporating the ideas into EPIC to be formatted in an easier manner to
