PCPs who know their patients well are often in the best situation to take the lead writing Care Recommendations if they are connected to the network via PreManage.
3. Mental/Behavior Health case managers can provide significant value to ED providers by including crisis plans, summary diagnosis and treatment plan information and care coordination notes in M/BH specific Care Recommendations.
4. Health Plan or MCO case managers may consider writing Care Recommendations when no PCP relationship has yet been established for a patient or when they can provide other information which could be valuable to ED providers, but which may not be available to other case managers working with the patient.
These are simply guidelines for EDIE and PreManage users to consider and the best approach to follow will always depend on community and patient-specific circumstances. Where possible, it is often very valuable for key stakeholders in a community to meet together to determine how to coordinate case management efforts for patients and who will play what role in writing Care Recommendations. These efforts can help streamline use of scarce case management efforts and provide community-wide clarity on roles for writing Care Recommendations and coordinating care more
