Mary was supported with personal care each day this quarter. Every morning Mary was verbally prompted to brush her hair. She sometimes requests assistance with this. She was verbally prompted to brush her teeth after eating breakfast. Staff monitored Mary to ensure that she followed through with the task and brushed her teeth thoroughly. When needed staff supported Mary with verbal prompts to wash her face or change clothes after eating. Staff monitored Mary's outfit choices to ensure that they were weather appropriate. Mary required verbal and at times physical prompts to apply an effective amount of deodorant after taking a shower and in the morning.
