The Robert Wood Johnson Foundation (RWJF, 2011), identified that collaborative decision making is in contrast to some traditional methods of care in hospitals. RWJF defined siloed care delivery as one of those methods in which “various professions miss the opportunity to improve efficiency and effectiveness by failing to collaborate sufficiently (para 2).” RWJF offers examples of the model working in complex care situations in which nursing leads the development of a plan of care, consults with specialists, and coordinates the patients care. Another example extends the team to include many disciplines, support services and external resources including social workers, translators and shelters.
