The planned changed process has two collaborators the social worker and the client they collaborate a plan and then execute a series of actions to enhance the client's functioning and well-being. The four phases of a planned change process are Engagement, Assessment, Intervention, and Evaluation.
Engagement is a phase that uses verbal and nonverbal skills that build rapport and trust with the client. This phase builds and establishes a relationship with a client. When building a relationship with a client you must always be aware of the way that you are talking and what language that you are using. This helps the client devise a plan, implement that plan. This helps the social worker monitor the client periodically and to determine if revisions are needed. In the intervention phase, there are two steps that the social worker will implement. The first stage is planning this stage is where the client and the social worker prioritize and identify each goal that may be broad or unattainable. This this helps the client and social worker discuss each goal and set specific terms for each one. The client documents each one and agrees upon the goals set forth with the social worker. The next step is the implementation step this step is putting the goals in motion. They review the contract and monitor the progress of the goals to see if they are realistic or if they need to be changed. A contract is another agreement that is a written contract. This contract helps the social worker and the client clarify goals, tasks and activities that determine who is responsible for each goal, task and activity. There is also a timeframe that is agreed upon to help stay on track with each one. Termination of the client system is based upon when the goals have been achieved or the worker ends the intervention or the goals are changed. Develop a game plan for follow-up or maintenance of goals. The Evaluations has two ways that is can take place. The first one is process evaluations this evaluates the relationship between the client and the social worker. This focuses on how they got along and evaluates how they worked on the intervention and how they did with the tasks and activities if they were completed. The next form is Outcome evaluations these evaluations this looks back at the termination of a social worker and a client to see if behavior of beliefs were the onset of the intervention of the working
