2.2.1 Lack of communication
There are multiple instances which depict lack of communication happening in this case, mainly:
• It took three months to get an appointment with a general surgeon, and there was no system in place to notify the patient about the waiting time or appointment confirmation.
• Greg was referred to a urologist due to failing health conditions, but there was no system in place to notify Greg or the primary care clinic that the urologist is out of town for an extended period.
• After the CT scans, Greg did not hear back from the primary physician or the walk in clinic about the results of the test for a week.

But, none of them had access to any previous medical history of the patient. Here in the example, Electronic Health Records (EHR) would have played a major role in enabling the doctors to access Greg’s medical history.
2.2.4 No access to patient health records
Greg himself did not have any access to his personal medical records. He called the labs for the result of his tests. This gap shows a missing patient portal solution. Patient portal solutions help patients to access their medical history using the internet. They also facilitate in scheduling appointments and tracking referrals sent to specialists.
2.2.5 No transfer of care responsibilities
As in the case of Greg who had a severe time-sensitive condition, there was no clear understanding of who is responsible for ownership of such situations. The continuity of care is crucial for such time-sensitive conditions which were absent in Greg’s case. eReferral system could have been used in this case to transfer all the vital information and care responsibilities to the specialists in such a time-sensitive condition.
