Valentine Center Observation

Due to the Court's mandated visitation imposed on my child, Natalie Cochrane, during the first week in August 2015, she has been irreperably harmed.

Per the Court's order, I dropped Natalie off at Mr. Watkins' home. She had been at Dr. Alturk's office 2 days prior, for severe constipation. The doctor had prescribed Miralax, twice a day, for five days.

I gave Mr. Watkins the prescribed medication and explained the dosage and instructions to both he and his mother, Christine Holt. Two days later I received a call from him, asking me to take Natalie, for the day, as he could not find a sitter and had to go to work. Patricia Bragen drove me over and
I picked her up. Upon our arrival at Ms. Bragen's home Natalie was screaming and obviously

We also contacted the police in Laurens County, SC. They advised, since the alledged incident occurred in New Jersey, the
Toms River police should be contacted. This was on August 11 and 12, 2015.

I was put in contact with Detective T. Grosse, of the Toms River SVU, and forwarded pictures of my daughter, including the emergency room paperwork. Dr. Stanton was to send a refferal for

the Valentine Center on Augsust 12, 2015. Unfortunately, he neglected to follow through. Several attempts were made to contact him, and all were unanswered. We then went to the hospital

to inquire more information about why the refferal had not been sent. We spoke with Trent, the nursing supervisor who then assured me that he would be taking the case into his own hands.

He just had to get permission from his supervisor to handle the case. Due to these delays, the referal was sent on August 17, 2015.

The valentine center contacted us, and due to a scheduling error on their part Natalie was not seen until Septmeber 3rd, 2015. The forensic exam was inconclusive, due to the delay.

Detevctive Gross received the paperwork from the Valentine Center, and due to the inconclusive diagnosis, he felt the case could not be persued since Natalie could not verbalize what did

His drivers licesnse is currently revoked due to a DUI he recieved earlier this year.

With that being said, his history of violence towards me, is highly alarming that he would be around my daughter. In December of 2014, he was holding Natalie, and had assaulted me while

holding her. I left with NAtalie and stayed at the safe house through providence house. Child Protective Services was involved because of the domestic violence that had occured infront of

natalie. During our stay at the Safe house, natalie and i both saw psychiatrists to help us deal with the emotional trauma we had endured.

I am asking the courts to seriously consider Mr.Watkins rights be terminated. I feel this is the only way i can provide a healthy, stable, nurturing life for natalie. I have always done what is best

for my daughter to ensure she has a good, safe, healthy environment to be raised in. I feel his continued involvement in her life would be a hinderence to this. MY daughter needs continuity

and stability, and I am begging the courts to assist me in

Dear Children’s Hospital and Medical Center Administration, I am a Doctor of Naturopathic Medicine, and I have worked at Children’s Hospital and Medical Center for eleven years. I am writing to disclose information that I believe demonstrates a misuse of locus of authority, a breach of the American Medical Associations Code of Medical Ethics, and a breach of Children’s Hospital and Medical Center Code of Conduct by Dr. Bud Shaw. After verifying the facts of Dr. Shaw’s story with the employees that were on duty at the time of the incident, I have chosen to share the information with the administration of Children’s Hospital and Medical Center. On October 15, 2015 in a personal conversation that took place in the cafeteria of Children’s Hospital and Medical Center between Dr. Sally Doe, Dr. Shaw, and myself; Dr. Bud Shaw shared the actions he took in the care of his daughter while she was a patient at Children’s Hospital and Medical Center in late September of 2015.…

According to Health Professional Council Authority (2015) conduct pathway “allows the council to manage notifications (complaints) that may constitute unsatisfactory professional conduct or professional misconduct”. Furthermore, according to Staunton and Chiarella (2013) “unsatisfactory professional conduct includes conduct that is substantially below the standard reasonably expected of a registered health care practitioner of an equivalent level of training or experience”. Based on the case study of Conyard [2015], the nurse was not able to recognise and respond to the deteriorating condition of the patient when the vital signs were in the red zone, in addition, the nurse was not able to document all the necessary assessments and plan of care. According to the Australian Commission on Safety and Quality in Health Care (2011), recognising and responding to deteriorating patient includes measurement and documentation of observations, escalation of care, activating rapid response system and communicating with the team about the clinical deterioration. The nurse would have recognised, responded and correctly followed the protocols by communicating with the multidisciplinary team immediately without delay.…
