Customer Satisfaction Survey Grey Memorial Surgery Center office manager, Jamie Hurt is conducting an online survey to measure our patient satisfaction with ENT Surgery Center. The purpose of the survey is to find out what kind of experience the patient had during their surgical ENT process. We want to know the quality of care that our facility is providing to our patient’s. This survey will allow us to identify our weaknesses and strengths within our facility. The survey will target the population of 21 to 75-years-old who has come through our surgical center. We will send the survey to every patient that provided us with an e-mail address. The survey should take about 10 minutes to complete. The instructions provide detailed steps for the patient to follow to complete the survey. We are offering the patient an incentive of $25.00 gift card to Walmart and Grey Memorial Surgery Center t-shirt to complete the whole survey.
At Grey Memorial Surgery Center, we see board range of surgery patients. The second section of the survey focusses on the general questions. We are wanting to know if the patient is a returning patient or a new patient to our facility. The purpose for these questions is to let us know generally how the patient feels about our facility, if this is the patient’s first surgery and their age range.
