The CDC states that approximately seven hundred and thirty-five thousand Americas have a heart attack every year, that’s one every forty-three seconds. Unfortunately, most hospitals still use paper charting to document these critical events. By understanding the practice issue, focusing on how to make change successful, discovering the tools needed for success, utilizing the best improvement models and quality measures, and calculating the necessary resources, quality improvement could be used for STEMI documentation nationwide.
Practice Issue The Acute Care Trauma Hospital sees approximately forty STEMIs a month from the Emergency Room. Currently, the process for STEMI documentation includes the primary RN paper charting then recharting

John Kotter implements his eight stages of transformational change to make quality improvement effective. These steps can be condensed to three major phases. Phase one, forming a sense of necessity by allowing the staff or those involved in the change the ability to form the new image. Second, allow the staff who want to be involved to form groups to establish and remove potential blockades. This staff will also develop victories that can fast-track other staff members drive to change. Last, allow the staff that is already enthusiastic about the improvements to institute a unit and/or system wide change. Without staff engagement and ownership of quality improvement, no change can happen. A system is only as strong as the lowest person on the totem pole. The model incorporates different levels that can help make the plan effective. The Plan phase involves deciding how to plan the project, who will run said project, what tools will the professionals need to succeed, and what reference point data should be composed. The Project Organizers will then discuss what is being cultured from doing this project, determine what happened when the plan was activated, and list any problems encounter. Next, the professionals will study the data collected and gauge what each measure showed. Last, the data will be comprised to determine what was gained from the project and aspects should be abandoned. A modified change will be made and the next phase of the project put into effect. The PDSA model can be integrated into this project by the following interventions; the project organizer would determine and plan the best week to pilot the project with the assistance of the nurse manager, staff members, nursing IT, and emergency medicine providers; this group would determine what data needed to be gathered such as increased or decreased STEMI door to procedure times; the group would meet every day during the pilot to talk about any problems encounter; at the conclusion of the project, the group would meet to study the data and evaluate the measures if door to produce times have improved; the plan would be put in place to act on the gains or
