Social Services: On 11/29/2016, Ms. Santana met with assigned Case Manager for the family ILP Document Review. Ms. Santana’s next ILP Document Review is on 12/13/2016. Ms. Santana is expected to meet with assigned Case Manager bi-weekly. Ms. Santana is in-compliance with the terms of her ILP. Case Manager asked Ms. Santana if she has any concerns or issues that she would like to discuss during the meeting. Ms. Santana stated no. Children were present at the time of the meeting.

Exit Strategy Plan: Ms. Santana’s Exit Strategy Plan continues to be on hold due to childbirth. Ms. Santana was reminded that once her exemption is lifted she would be expected to adhere and follow the terms of her Exit Strategy Plan, which will consist of obtaining employment, savings, applying for low income apartment, and/or exploring

Rules and Regulations: Ms. Santana was reminded of the Rules and Regulations of the facility as well as the curfew. Case Manager reminded Ms. Santana that if she violates facility's curfew, rules, and/or regulations a warning will be issued for the violation. Ms. Santana stated that she is aware and she understands. Health and Safety: Ms. Santana was reminded of the weekly room inspections and she is to allow staff into her unit to conduct room inspections. Ms. Santana reminded that she is to maintain her unit in a clean, hazardous free condition. Ms. Santana was reminded that fire drills are conducted monthly and that she is expected to participate. Case Manager explained to Ms. Santana that if she failed to abide to any health and safety rules of the facility a warning will be issued for non-compliance.
