On 8/12/17 CM met with the client to complete Bi-Weekly ILP Review. Client was dressed in proper attire for the weather. She was cooperative and friendly in the meeting.

The following subjects were discussed: MEDICAL: client report no medical issues this week everything is under control. Next upcoming appointment is scheduled on 8/21/2017 with Dr. Aaron Pearlman. Client also reported her last PT was completed last month. She reports no changes in medications.

Housing: Client Linc IV certificate was approved on 08/10/2017 and will expired on 11/08/2017. Client monthly contribution is $174.00.

CM provided client with Housing Search Log and advise the client to view at least 3 apartments per week. Client was informed that she need to attend to all scheduled housing tour. …show more content…
Entitlement: Client is receiving. SSD in the amount of $580.70 and submitted an update Award Letter.

Rules and Regulations: Client is compliance with the shelter rules and regulation. CM mention to the client to adhere to the 10pm curfew.

Client also reported she is interested in teaching other residents how to do sugar flowers. CM refers the client to speak with the Recreation Supervisor. CM uses Google Translate to translate the Bi-Weekly ILP Review. Client agreed and signed. Next Bi-Weekly ILP Review is scheduled for
