MIH/CP staff receive referrals from EMS personnel via the EMS EHR system ESO Solutions. EMS staff select an option in the EHR that identifies a patient in need of MIH/CP services. These referrals come via email to the MIH/CP in an automated report each day. The EMS crew also has the patient sign a release form that provides permission for the MIH/CP to contact the patient. The form also contains a Health Insurance Privacy and Portability Act (HIPAA) release. Appendix E contains the release form. Training must address the following groups: MIH/CP, physicians, nursing staff, and IT support staff. Training will consist of how to log in to the Skype program and conduct the appointment. Further training will include the physician talking to a simulated patient so that familiarity and comfort with a remote interview occurs. Next, the training addresses physician and MIH/CP interaction. Tasks such as vital sign gathering and physical exam functions occur, and data is relayed to the physician. Trust must develop between the physician and the MIH/CP. Nursing staff will take part in the training so that they are comfortable with all aspects of computer functionality, along with carrying out a remote appointment much like the physician. Last, the IT staff must understand how to support the technology side of a telemedicine appointment. These staff must be familiar with the mobile gateway installed on the MIH/CP vehicle, along with other methods the MIH/CP may use to obtain internet connectivity.
