MG-2s In Physical Therapy Case

I am in receipt of an MG-2 form filed by Dr. Czajka on 08/23/17. The doctor is requesting back therapy, two (2) to three (3) times a week for six (6) to eight (8) weeks. The Special Funds approved the MG-2 on 08/24/17. It is my understanding the doctor is going to ask for a years worth of physical therapy as he has done in the past. This MG-2 is good for a eight (8) weeks and he certainly can renew it after you have done the eight (8) weeks of physical therapy. You might want to talk to him about requesting more then eight (8) weeks otherwise he is going to be constantly filing MG-2s in this case.
