Considering this patient scenario, my choice of actions would involve four different steps, underlying one of the principles of ethical decision making, the respect of autonomy (patient’s preferences), as my primary moral goal.
3.1) Communication between patients
Firstly, I would articulate patient preferences through communication and consultation. The communication process would simply be carried out by listening to what patient identifies as her wishes, and focus on the expressed wishes and values of patient, for example with regards to specific medical interventions. Additionally, I would assess patient’s understanding of the medical indications relevant to her case of diabetes mismanagement. Assessments would include evaluation on information quality, sufficiency and patent’s health literacy. It is equally important to ensure when patient is prescribed to a medical treatment, the decision is made in a truly voluntary manner without constrain. The patient’s best interest should always be the centre of healthcare decisions, which is derived directly from the ethical principles of patient autonomy and beneficence.

Although patient is still the chief stakeholder in this nutrition therapy, the concerns of family members could also have significant impact towards the effectiveness and efficiency of an intervention strategy. More important, the patient’s family is often the most compelling element in building one’s faith and culture, especially when dealing with issues of serious illnesses. Besides, the whole communication and consultation process should also be recorded in confidential written documents.
