The issue was that there was a decline in providing medication teaching to patients. Patients would ask other disciplines about medications such as indications and side effects. Disciplines who do not have the basic knowledge of medications was not able to answer patients’ questions. As a team, the agreement was to add a simple basic teaching tool for medications in our software to share with patients. Further teaching, assessment, or evaluation required a licensed nurse. The team’s effectiveness was strong as evidenced each disciplines’ willingness to learn and participate in the plan. Team building requires active participation and clear communication towards a common goal (Cooper-Duffy & Eaker, 2017).
