Regarding failure of service to service user, one scenario is from my previous placement experience. Mr England(not real name), 45 years old, Elective Appendectomy, no past medical history, admitted in pre-op day surgery ward in night for following morning operation. Due to emergency events, the operation postponed and then slotted in second day morning. The operation finished before lunch time and when I was on night shift with mentor on the same day, his post operation EMS score was raised from handover 0 to 3. He also made a complaint of “starved”. Due to complex situation, patient had been in hospital another three days longer than initially 24 hours post op discharge plan.
In this case, the interprofessional Collaboration should have been a rational strategy to achieve the certain goal: patient safely

Managing fasting time, fluid balance and ensuring patients do not become dehydrated or malnourished are the key aspects to consider. To achieve these, most importantly, using quality and service improvement tools to analyse (ICO, 2011), patient-centred approach should be performed. A thorough holistic assessment of patient needs should reflect in his care pathway. Despite best-practice guidelines, preoperative fasting times remain excessive (Roberts, 2013). Therefore, from the first moment of patient admission, a fasting plan of care and associated assessments should be implemented, documented and updated. The nil by mouth guideline should be kept to minimized time. As Hamid, (2014) suggested a patient centred approach to pre-operative fasting should be to improve patient wellbeing and satisfaction. Mr England should have been educated and fully informed about his fasting and operation to avoid unnecessary prolong fasting time when surgery was delayed. It could promote a smooth and rapid
