The definition of patient- centered care is being able to recognize the patient as the starting place of control and being able to manage the care based on the patient’s needs. The provider needs to include several things in order to appropriately cater to the patient’s needs such as the patient’s preferences on how they are being cared for, as well as their values and culture. The nurse needs to know how to coordinate the patient’s preference with their plan of care. Detailed instructions about a culture or religion need to be discussed in an interview and a specified plan of care needs to be incorporated. The patient’s preferences should be communicated with all members of the health care team. Likewise, everyone needs to be sensitive and respect the patient’s requests. The nurse needs to be compassionate and support the patient's values even if they differ from their own. They need to know how to talk with the patient and give them the information and education that they might need. The nurse also needs to know how to give …show more content…
All members of the team need to be able to have adequate communication, respect each other, and compromise to make the best decisions for the patient. Each member should know their role on the team in addition to the other member’s role. Every member of the team has their own special abilities that are needed. “The essence of collaboration involves working across professional boundaries” (Liedtka & Whitten, 1998), so each member of the team needs to be able to adjust to the way the other team members do things. It is not always going to be a happy workplace, not everybody is going to agree on the same plan of action; but if it is the best option for the patient then a compromise must be met. The skills and knowledge of the whole team is greater than just one

Also, the trend of the medical care is to demonstrate treatment efficacy by means of quantifiable outcome measures. Thus, the variety of interventions provided to people with cancer and survivors of cancer should be accompanied with appropriate measures. Some health care models were used to fill the gap such as “Patient Centered Care”. The multidisciplinary team approach is a collaborative process in which all of the professionals work together to treat the patients while fulfilling the needs and wishes of the patients and the patient's family. Marissa, I agree with your post. Patients often know when they are receiving very great care, however it is often the health care team that do not keep everyone informed of what will take place and what the procedures will and will not work and why. It is often that everyone that is inclusive of the team is not informed such as the patient's family. I often hear patient centered care, but as Dr. DiGola states, keeping everyone inform it is PFCC, Patient, Family Centered Care, which includes everyone from inception to discharge, which keeps everyone informed. Which results in better outcomes and better satisfaction surveys (DiGiola, T., 2013).…

Introduction The purpose of this paper is to uncover and evaluate the role of nurse advocate in the delivery of patient-centered care. “Patient centered nursing interventions are a response to the needs of patients, and the coordination and individualization of care, skill, and expertise” (Esmaeili, Cheraghi, & Salsali, 2014, p. 3). Patient-centered care allows for better distribution of time for the delivery of care, lower treatment costs, shorter hospitalization stays, and earlier discharge, all of which ultimately improve patient satisfaction rates (Esmaeili, Cheraghi, & Salsali, 2014).…
