Presenting Problem: Admitted for evaluation and stabilization of Bipolar symptoms, increased Depression, Suicidal Thoughts and wanting to die. She is being admitted on a voluntary basis after ingesting Lamictal, Concerta, Seroquel and Vistaril. She states she was not trying to commit suicide, that she wanted to just go to sleep. She reported having chroncproblems with sleep, depression and anxiety. The ingestion of these medications caused Cardiopulmanry Arrest. She was resuscitated and admitted to ICU. She has a Hx of polysubstance use with alcohol, marijuana and cocaine. Reported while at Poplar Springs shewas calm and cooperative on the unit and interacts well with her peers and staff and is medication compliant. She also denied SI/HI while
