Wound Care Reflection

On 11/10/17 I met with Ms. Perez at the Chesaning rehab center for a team meeting. Ms. Perez was in her hospital bed. She continues to have weeping edema to her right arm and the lower extremities. She had a recent appointment with, Dr. Malik wound doctor. He was very happy with the progress of the abdominal fold wound. The wound measures 12 by 3 by 2 inches at this point. Wound care is done daily. She will not see Dr. Malik again until January. Ms. Perez also saw Dr. Wolohan orthopedic surgeon. The fractures are healed and she may now bear full weight on her legs. He advised her to start working with physical therapy. Physical therapy reports that she has refused therapy. They will come to her room 3 to 4 times a day to get her up and …show more content…
They need to use a Hoyer lift now to get her up and out of bed. She is a maximum assist with any therapy now. A social worker reports she spoke with Ms. Perez about her refusal to her physical therapy. They have been trying to set goals for her. The rehab psychiatrist came and saw her and recommend increasing her anti-depressant and she refused. Ms. Perez said she did not want to be dependant on that. After everyone gave report we spoke very frankly with Ms. Perez about if she wants to get out of the facility and be able to go home. She said she does. She will think about increasing her anti-depressant for a short time if that would help. She also agreed that she would get up and into a chair and work with physical therapy. We stressed the need for good blood flow, to avoid skin breakdown and also avoid blood clots and pneumonia. She said she would try harder. Mr. Perez was also at the team meeting. He said he wants her to come home and be
