On 5/17/17 case worker received a call from Mrs. Nuckols asking if integrated behavior health (IBH) mail out a written correspondence to DHR. The worker told Mrs. Nuckols that this point, she has not received any correspondence. Later that evening, the case worker received a letter from IBH. The letter addresses Mrs. Nuckols 3 diagnosis. Mrs. Nuckols currently diagnosed with Bipolar Disorder, Generalized Anxiety Disorder, and Post-Traumatic Stress Disorder. Mrs. Nuckols is takeing prescribe medications help with the disorders listed above.
