Mr. Singer's Case Summary

Mr Singer is a 61 year old male who presented to the ED with uncontrollable behavior at his living facility. He does not presents to ED with aggressive behavior or agitation. Mr. Singer denies suicidal ideation, homicidal ideation, and symptoms of psychosis to nursing staff. At the time of the assignment Mr. Singer presents calm and cooperative. He states today he became angry because one of the nurses tried to take away his pens, that he was given to by another nurse as a Christmas gift. Mr. Singer denies current suicidal ideation, homicidal ideation, and symptoms of psychosis. He denies current or recent feelings of depression. Mr. Singer reports he does not remember what actually happen after he became angry today. Per BrookeStone Haven staff this incident happen between the hours of 6am-2pm or first shift. They report their staff members tried to given Mr. Singer a bath and e began to yell, "Y'all don't tell me what to do." He pushed a staff member against a wall and twisted her wrist. Per BrookeStone Haven staff nurse has a few scratch marks on her hands. He was seen pacing, cussing, and saying, "I will hurt you. I will mess somebody up." He was exhibiting these behaviors while flailing his arms. Per documentation Mr. Singer has a history of Moderate MR, Bipolar 1 disorder with mixed features, and Major Neurocognitive Disorder due to Alzheimer with behavioral disturbance. …show more content…
TACT consulted with Dr. Snyder it was recommended to refer for inpatient hospitalization for safety and stabilization. TACT assisted the ED doctor in completing IVC paperwork. TACT will search for appropriate
