Jessica is a 21yo, G1 P0, who was seen for an ultrasound evaluation and consultation for FTS. The patient has a history of opiate use disorder involving heroin addiction. She has used this off/on with the father of the baby and has actually used it recently. She states that she recently entered your Subutex program and is now on 8 mg Subutex b.i.d. and believes that she is doing better. It does appear that she has been off/on treatment in the past and relapsed numerous times and therefore has a complicated opiate use disorder history. She does smoke cigarettes at ½ pack per day. She was further counseled on cessation. She also has issues with anxiety and has been on Klonopin and Prozac but she states that she discontinued both of these with a positive pregnancy test. The patient states that she is interested in continuing her PNC in your office with your Subutex provider. She might be a patient who would be difficult to wean due to her longstanding history of numerous relapse. In addition, there are issues with the father of the baby who is probably continuing to use heroin off the streets and therefore, again, this is a complicated psychosocial family history.

Upon completion of the ultrasound and assessment, all of her questions were answered in detail. Again, we will forward the results of the FTS to your office upon their return. If this returns low-risk, the plan will be to perform a good fetal anatomy ultrasound at 20 weeks gestation. If you would like us to perform any of her future scans, please feel free to reschedule her with us.
