Daimarkus is a 27-year-old AA male who presents to CRU 2 from UPC. He is on ACOT for PAD. Patient stated he was in prison for 3 years for attempted armed robbery, he was released on the 04/14/17. He was recently discharged from DVH. According to his Psy eval from UPC, he was stayed on COT whiles at DVH. Lifewell South Mountain is his outpatient treatment agency. He is alert and oriented 4x. He presents as calm, appropriate and cooperative during the intake process. Patient denies DTO/DTS. He denies AH and VH, but stated he was hearing voices yesterday. He will benefit from meeting with the provider.
