The patient is a 20 year old male who presented to the ED via LEO under IVC with alleged suicidal ideation with multiple plans. Per documentation the patient threaten to cut his throat, jump off a bridge on 85, or get killed by LEO when they came to get him. The patient reports conflict between his mother and him is the contributing factor to his distress. Patient denies suicidal ideation, homicidal ideation, and symptoms of psychosis. per documentation patient states, " my mother got mad at me about a bill she got for stealing my disability check and she told me to go kill myself." He denies feeling depressed. Patient does not appear to be exhibiting signs of agitation, aggression, or responding to internal stimuli.

At the time of the assessment the patient reports vaguely remembering the events today due "blacking out". The patient states, " the only thing I really remember is the argument with my mom, then I got mad and blackout." The patient reports walking outside and speaking with his dad about the matter, however during his conversation his sister said something he didn't like and assaulted her. Patient admits he hit her in the face, The patient appears guarded during the time of the assessment. The patient admits to smoking marijuana daily for the past two years. Patient states, " it helps me relax and calm down."

Due to an suicidal ideation with a plan, history of attempts, poor insight, poor judgement, poor impulse control, patient does meet criteria for IVC and inpatient hospitalization. TACT consulted with Dr.Keith and it was recommended to refer for inpatient hospitalization for safety and stabilization. TACT assisted the ED doctor in completing IVC paperwork. TACT will search for appropriate
