Mr. Wilson and I sit down to start his assessment. I explain what the assessment is for and confidentiality. We first start by obtaining personal information. He gives me his full name, his address, and talks about his wife who passed away. While talking about his wife I asked if he had other family members. Mr. Wilson told me he had a daughter and a pet dog. He told me he used to work as a farmer but know he just enjoys siting with his dog on the porch. I asked if he had any close Nabors but he said his closest neighbor was about five minutes away and his daughter lived about thirty minutes away. I then proceeded to ask Mr. Wilson about his typical day. He told me every morning he gets out of bed fixes a cup of coffee and a bowl of cereal and reads the newspaper. After breakfast he takes a shower and brushes his teeth. He then proceeds to walk his dog and check on things around his farm. He said he has a small garden that his neighbor’s son helps him with. He said around lunch time he makes a sandwich and usually takes a nap. When he wakes up he straights up the house. At the end of Mr. Wilson’s day he has something simple to eat, like a frozen meal his daughter keeps the fridge stocked with. He then watches TV or reads until he goes to bed. I then asked Mr. Wilson if he had any hobbies he liked to do. He said he enjoyed fishing and goes to church when his daughter could pick him up.
