This is a very simple component of a treatment plan taking the stated problems or conditions and converting them into an action statement indicating what needs to be done to reach the goal.



Objectives are step by step procedures involving behavioral change and then practice to reach the goal.

OBJECTIVE

Problem Statement: Illegal Stimulant Use


Objective: Keep a log of all thoughts related to stimulant use

Examples of Problem Statement and Objective

Problem: "I'm addicted to drugs, it's something that is controlling my life "


Goal: "I have to figure out a way to make recovery work."


Objective: Client will identify triggers that can lead to relapse.


Intervention: Counselor will assist in identifying triggers, educate about high risk situations, and address potential ambivalence through the use of MI and CBT. Counselor will assist client with expanding social support that includes non using friends, generating ideas for social activities, and educate the client about benefits of a healthier lifestyle.

Problem Statement, goals, and objectives

A counselor specifically identifies and rank order the clients presenting problems. The counselor then relates the client's problems to the clients needs.



For example Leslie might relate that in her assessment Victoria presented three problems: the first problem was her abusive alcohol the second was her misuse of prescription medications and the third with her lack of education. Victoria needed to abstain from all youth of mine altering substances, take her prescribed medications appropriately, finish her ged, and then enroll in a post-secondary training program.

Identify and rank problems based on individual client needs in the written treatment plan.

A counselor works with the client to develop short-term and long-term goals while obtaining client input. It would be appropriate for the counselor to explain the treatment plan format to the client suggesting specific examples of the client problems, the goals problem both immediate and long-term, and the objectives for reaching the goals.



Short-term goals can be achieved by the end of the planned period of treatments.



Long-term goals extend beyond treatment, whether it is lifelong or some period less than that.
