After the evaluation, the case manager and therapist will talk about the client’s implications, and discuss recommendations based on each patient's strengths and accessibility (language/communication skills). Next, a treatment plan will be formulated that serves the client best interest with a schedule for therapy visits and group sessions with other youth if applicable. The treatment plan will be created with the psychiatrist, case manager, and therapist with input from the client and their guardian if the client is not able to make sound decisions for themselves. Outside agencies involved with the client’s treatment will be discussed and documented. Medications prescribed for the client will be discussed and documented. Medication and other agency involvement will help to ensure nothing is overlooked or redundant for client’s treatment plan. While mental health is the focus of the program, other factors hinder the deaf and hearing impaired community, such as communication barriers, which is why Silently Evolving offers ASL assistance. This is why everyone in the program will have to be fluent in ASL and ASL learning services will be provided. The group sessions provided give both families and clients times to interact with people that face similar circumstance so that they know they are not alone in their fight and have people that they can potentially build relationships with. Evolving Silently is a program within the DREAM clinic, within Kennedy Krieger Institute.
